NAOSH Achievement Award Nomination Forms for 2008
Nova Scotia Chapter Canadian Society of Safety Engineering 2008 Outstanding Achievement Award: Occupational Health and Safety Professional
Presented to an Occupational Health and Safety Professional in Nova Scotia who has made an outstanding contribution to workplace health and safety through significant personal leadership, initiative, personal commitment and volunteer activity above and beyond the norm required by his/her employment. If the effort represents many years of dedicated service, it may be awarded as a Lifetime Achievement Award.
Please note that persons nominated for this award category must have a post-secondary academic qualification in an occupational health and safety discipline or meet the education and experience requirements of a professional certifying organization. Nominations of other persons should be made in the “Individual” award category.
The award will consist of a citation, an engraved plate and a continuing education prize.
Nomination of an individual to be considered for this award may be made by their employer, co-workers, Joint Occupational Health and Safety Committee, or by an occupational health and safety organization.
The nomination should clearly identify the reasons why the nominee is worthy of the award, including detailed documentation of achievement and letters of support. Please see the next page for a Nomination Form with additional detail.
The Awards Committee will select the nomination they feel best exemplifies the purpose of this award. The decision of the Awards Committee will be final and the Awards Committee will not enter into any correspondence regarding its decision. The Awards Committee reserves the right to withhold the presentation of this award if, in their opinion, no suitable nomination is received.
